Role:
We are looking for experienced GPs eager to work in a fast-paced, patient-first service. This role is for our @Home service, which provides care for patients across Lambeth & Southwark. The service focuses on supporting early hospital discharge and hospital avoidance, managing patients who require significant input beyond traditional community care but do not yet need inpatient admission.
Clinical Responsibilities:
The post-holder will provide medical care to patients within their homes, managing a caseload and ensuring the highest standards of care. You will work closely with SELDOC’s multidisciplinary team of Advanced Care Practitioners (ACPs), nurses, and secondary care physicians. The role involves:
- Participating in daily virtual ward rounds with secondary care physicians.
- Undertaking home visits (alone or jointly with nursing staff) to assess and treat patients, including those recently discharged or at risk of hospital admission.
- Setting up IV lines, performing cannulation, and administering intravenous antibiotics as part of acute patient care.
- Developing and managing care plans in collaboration with nursing staff and other healthcare professionals.
- Acting as the overall medical lead for the @Home service during your shifts.
Skills & Qualifications:
- Full GMC registration and experience as a GP within the NHS or private sector.
- Experience in setting up IV lines, cannulation, and administering intravenous antibiotics.
- Ability to work within a multidisciplinary team, co-managing patients with secondary care clinicians.
- Strong clinical leadership and decision-making skills.
Desirable Experience:
- Recent experience working in A&E or acute medical intake.
- Familiarity with using EPIC or similar electronic patient record systems.
Location:
@Home community bases. (You may also be required to attend other locations.)
Hours: Bank Role
Salary: Up to £105 p/h (Outside IR35)
