Lead Advanced Nurse Practitioner

Are you an experienced Lead Nurse Advanced Nurse Practitioner (ANP) with General Practice and management experience? 

We are recruiting for a Lead Advanced Nurse Practitioner (ANP) with General Practice, management experience and good knowledge of clinical governance to take on the clinical lead responsibility for the Junction Health Centre (JHC) nursing team. The JHC is a high QOF achieving and Good CQC rated service located in Clapham Junction. 

Run by Practice Plus Group, The Junction Health Centre is a family-friendly NHS surgery with 8,600 registered patients and a Wandsworth Enhanced Access Hub service. The services provide the community of Wandsworth with treatment and advice for a wide range of minor illnesses and injuries and long term conditions. We also provide the usual range of primary care services including immunisations, women’s health, health screening etc.

We have a well-established team of approachable and supportive clinicians comprising of - GPs, ANPs, Pharmacists, Paramedics, Practice Nurse and Health Care Assistants.

The Practice is an active member of the Primary Care Network and benefits from additional clinical staff offered through the Additional Roles Reimbursement Scheme.   

This unique role presents an excellent opportunity for an experienced ANP. As part of The Junction Health centre, We can provide you with the support and the tools for the role and offer you the chance to develop and diversify your career. As the Lead ANP, you will be responsible for supporting the Management Team with clinical effectiveness activities, clinical governance, CQC readiness and service improvements.

The Lead ANP will be given 4-6 hours of weekly protected management time for these duties. 

The vacancy is a permanent contract with the option to work between 24hrs - 37.5hrs p/w. This comes with a FTE salary of £65,000 per annum.

The working pattern is weekdays with the option to work on weekends. Our service is open 8am-8pm Monday-Sunday.

Principal Duties and Responsibilities

The following are the core responsibilities of the Lead Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Organise and co-ordinated the provision of nursing services for the practice to include the delegation of duties to other practice nursing staff as appropriate
  • Provide nursing treatment to patients in participation with doctors or independently to agreed protocols
  • Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to doctors as necessary
  • Advise patients on general health care and minor ailments
  • Develop, implement and embed health promotion and wellbeing programmes
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patient’s health
  • Referral / sign posting to appropriate health professional / services.
  • Provide patient information and education.
  • Ensure full and accurate records are maintained and confidentially held.
  • Maintain confidentiality of information regarding patients and staff at all times.
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Provide wound care
  • Set standards for patient care based on best evidenced based practice.(i.e. clinical protocols).
  • Develop / maintain advanced clinical skills. i.e. x-ray interpretation, suturing, plastering , nurse led services
  • Provide travel medicine services
  • Request pathology services as necessary
  • To develop and maintain competencies associated with the role, ensuring yourself and more junior staff are working within own limitations / scope of practice.
  • Work at all times within the NMC Code of professional practice.
  • Personally responsible to maintain NMC registration and professional development in line with the expectations of the role.
  • Deliver opportunistic health promotion where appropriate
  • Have a high level of clinical expertise and leadership skills to give support and supervision to the ANP’s, Nurses and HCA’s as a clinician and a leader.
  • Work with the Local Medical Director and Service Manager to undertake delegated tasks in the ANP’s, Nurses and HCA’s management.
  • To assist the Registered Manager in managing CQC assessment visits and providing self assessment data as and when requested.
  • Provide, organise and implement clinical supervision in accordance with Company policy.
  • Participate in the review and development of Patient Group Directions for non-prescribing staff
  • Medicine management audits and advice
  • Administer and provide treatment within local guidelines / protocols.
  • Maintain standards of infection control within the unit and during clinical care.
  • Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
  • Support the clinical team with all safeguarding matters in accordance with local and national policies
  • Act as the audit lead, effectively utilising the audit cycle
  • Act as the IPC lead for the practice, ensuring compliance, undertaking audit as necessary
  • Monitor and ensure the safe storage, rotation and disposal of medicaments
  • Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff
  • Maintain an effective nursing staff rota ensuring all clinics are staffed appropriately
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the organisation
  • Develop an area of specialist interest, taking the lead within the organisation
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

Essential Qualifications & Experience:

  • Registered Nurse
  • Independent prescriber
  • MSc qualifications in clinical assessment
  • Clinical skills qualification at degree level
